The City of Mill Creek is an acceptance agent for the United States Passport Service.  You may submit your application and obtain your photographs for a United States Passport at City Hall offices during the hours listed below. You do not have to reside in Mill Creek - all are welcome.

We are open to process passport applications during the following hours:

Monday - Friday 8:00 a.m. to 5:00* p.m.
Saturday 9:30 a.m. to 5:00* p.m.
(Morning hours temporarily expanded due to increased demand)

*Application Submission

Traditionally, the last application is accepted 15 minutes prior to closing. However, in the event we receive a rush of customers toward the end of the day, passport technicians may remove the number machine early. We apologize for any inconvenience this may cause.  Call 425-921-5743 before arriving to verify applications are still being accepted.

All applicants must be present at time of application. Applicants under the age of 16 must be accompanied by both parents. Payment must be made by check or money order. 

Things to Remember

  • All questions on the application are about the person applying
  • Minors should use someone other than a parent as an emergency contact
  • Social security number is required for all applicants
  • Mother’s maiden name is required
  • Spouse’s maiden name is required
  • Use black ink only on your application
  • We only accept check or money orders for the passport payment
  • Your emergency contact should not be someone you travel with

Current Fees
  •  Adult Passport Book - $110*

  •  Adult Passport Card - $30*     
  •  Minor (under 16) Passport Book - $80*
  •  Minor (under 16) Passport Card - $15*

  •  Processing Fee - $25
  •  Expedite Fee - $60*
  •  Passport Photos - $16
There are two separate payments. Passport and expedite fees are paid to the Department of State*. Processing fees and passport photos are paid to the City of Mill Creek.

* These payments must be made by check or money order.

Minor Passport Processing
Minors (under 16) must apply in person. You will need the following:
  • Both parents
  • Birth certificate (original/certified copy)
  • Check or money order
  • Expired passport (if available)
  • Recent Photo **
  • Social security number
  • Form DS-11

Adults who were under the age of 16 when their last passport was issued must apply in person. You will need the following:
  • Check or money order
  • Expired passport
  • Form DS-11
  • Recent Passport Photo**(w/in 6 mo)
  • Identification
  • One parent is required for applicants who are 16 or 17 years old

Passport Renewal (Adults)
All adult renewals are done through the mail. You will need to mail the following:
  • A certified copy of your marriage certificate or court order if your name has changed
  • Check or money order (see fee chart above)        
  • Form DS-82
  • One recent passport photo**
  • Your expired passport

Adult renewals are only processed in person if your most recent passport has been damaged, lost, or stolen, or if it has been expired for more than five years.

**You may bring a passport photo (taken within the last 6 months) with you or get one taken on-site for $16.00.

Processing Times
According to U.S. Department of State, processing times for routine and expedited applications are as follows:
  • 6-8 weeks for routine processing
  • 2-3 weeks for expedited processing

Passport Fraud
A U.S. Passport is an extremely important document.  With this in mind, extra precautions are taken to ensure the integrity of the passport application process.
