All applicants must be present
at time of application. Applicants
under age 16 must be accompanied by both
parents. A U.S. Passport is an
extremely important document. Extra precautions
are taken to ensure the integrity of the
passport application process.
Passport
fraud is a felony, punishable by up to ten
years in
jail and/or a $250,000 fine.

What are the new passport
regulations and how do they affect me? |
Effective January 23, 2007, ALL persons, including U.S.
citizens, traveling by air between the United States and
Canada, Mexico, Central and South America, the Caribbean, and Bermuda are required to present a valid
passport, Air NEXUS card, or U.S. Coast Guard
Merchant Mariner Document, or an Alien Registration Card, Form I-551, if
applicable.
As early as Summer 2008, ALL persons, including
U.S. citizens, traveling between the U.S. and Canada, Mexico, Central and
South America, the Caribbean, and Bermuda by land or
sea (including ferries), could be required to present a valid
passport or other documents as determined by the Department of Homeland
Security. While recent legislative changes permit a later deadline, the
Departments of State and Homeland Security are working to meet all
requirements as soon as possible. Ample advance notice will be provided to
enable the public to obtain passports or passport cards for land/sea
entries.
The passport requirement does NOT apply to U.S. citizens traveling
to or returning directly from a U.S. territory. U.S. citizens returning
directly from a U.S. territory are not considered to have left the United
States and do not need to present a passport. U.S. territories include the
following: Guam, Puerto Rico, the U.S. Virgin Islands, American Samoa,
Swains Island, and the Commonwealth of the Northern Mariana Islands.

What is the difference between a US
Passport Book and the new Passport Card? |
The passport card will facilitate entry and expedite
document processing at U.S. land and sea
ports-of-entry when arriving from Canada, Mexico,
the Caribbean and Bermuda. The card may not
be used to travel by air. It will
otherwise carry the rights and privileges of the
U.S. passport book and will be adjudicated to the
exact same standards. You can apply for one or both,
it's your choice.
The Department of State is issuing this
passport card in response to the needs of border
resident communities for a less expensive and more
portable alternative to the traditional passport
book. The card will have the same validity period
as a passport book: 10 years for an adult, five for
children 15 and younger. Adults who already have a
fully valid passport book may apply for the card as
a passport renewal and pay only $20. First-time
applicants will pay $45 for adult cards and $35 for
children.
To facilitate the frequent travel of U.S. citizens
living in border communities and to meet
Department of Homeland Security
operational needs at land borders, the passport card
will contain a vicinity-read radio frequency
identification (RFID) chip. This chip will point to
a stored record in secure government databases. There
will be no personal information written to the RFID
chip itself.
Note: A previous passport book holder, eligible
to use
Form DS-82, may apply for a passport card as a
Renewal.
First time applicants for a U.S. Passport, and
those not eligible to use the DS-82, should apply
for a passport card using
Form DS-11.

What
documents do I need to apply for a passport book or card? |

$75 Persons age 16 or older
$60 Persons under age 16 (must be accompanied by a parent or
legal guardian)

Two separate
payments must always be made:
- The first
payment which covers the Application Fee and Expedite Fee (if applicable), can
only be paid by check or money order made payable to: U.S. Passport Services.

payment, which consists of the Execution Fee, photos (if taken here) and postage
fee (for expedite service only) may be paid by cash, Visa/MasterCard
or check made out to: City of Mill Creek.

How long does it take to get a
passport book or card? |

Passports are generally received 4 - 6 weeks from time of application. |

What if I need a passport
book or card in a
hurry? |
Passport applications can be expedited for an additional fee. Expedited
passports are received within 2-3
weeks. If you are
leaving in less than 14 days, you may contact the
Seattle Passport Agency Office at
1-877-487-2778 to make a special appointment.
Proof of travel is required.

Can I track my passport
book or card application online? |
|
Yes, you may track the status of your application online by visiting
http://travel.state.gov/passport/get/status/status_2567.html using a
standard browser. You can securely enter online your last name, date of
birth, and the last four digits of your Social Security Number to receive that
information. All personal data will then be deleted from this system.

Yes, there is no age minimum. |

Can I renew my passport by
mail? |
|
YES, you are required to renew by mail under the following circumstances:

you already have a passport that is not damaged; and
-
you received it within the past 15 years; and
-
you were at least age 16 when it was issued; and
-
you
still have the same name as in
passport (or you can legally
document your name change)

What if my passport has been
lost or stolen? |
Report the loss on Form
DS-64 when you apply in person for a new passport. If you are abroad,
immediately report the loss to local police authorities and the nearest
embassy or consulate.

What if I
have had a name change? |
|
If your most recent U.S. passport was issued less than one year ago, use form
DS-5504. Include certified evidence to support a name change or other
change in descriptive data and two new passport photos and mail it to address on
form to get a new passport free of charge.
If your valid U.S. passport was issued more than one year ago, use
form DS-82. Include certified evidence of name change or other change in
descriptive data, two new passport photos and a check (see fee table), and mail
it to address on form.
